The State of Georgia REQUIRES the following documents to process an IRP TAG. NO EXCEPTIONS

_Power of Attorney sheet(s) signed, notarized, and returned

_3 Proofs of Residency: Driver's License/Title or Title Receipt-Utility Bill (Gas, Electric, Water, or Land) Also accepted are Personal Tax Returns, Property Tax Records, Personal Vehicle tag/title receipt, addresses/name must match on all 3. NO PO BOXES

_Certificate of Insurance, card or fleet card *with full VIN and expiration date

_2290 unless truck has been purchased less than 60 days ago or gross weight is under 54,999 lbs.

_Corporation documents are also required if tags are desired in your company name.

*If you are leased on with someone, the state of Georgia also requires:

_Lease Agreement with the company you are running under (1st page, truck info page, and signature page)

_Carrier MCS15- Motor Carrier Identification

_Carrier Certificate of Insurance, card of fleet card (With full VIN and expiration date)

_Copy of front and back of Title or Title Receipt in personal or company name (titled in Ga.)

(if leased, you will need an equipment lease agreement NOTARIZED)

_Payment: Tag fee ($76 for first truck and $26 for each additional truck
